Application
Form
The online application will be electronically submitted to the Office of Admissions and then forwarded to the appropriate academic department. In addition to the on-line application, a $50 non-refundable application fee must be electronically submitted.
Credit Overloads
Form
Credit overloads allow a student to register for more than the university maximum credit load for the student's level (Graduate - 15).
- Credit overloads with the maximum credit load between 16-18 credits can be entered by the student's department via ARIESweb. Visit the Credit Overloads page on the Registrar's Office website for instructions.
- Credit overloads with the maximum credit load above 18 credits must be directed to the Graduate School for approval. The student's advisor will need to send an e-mail to gschool@grad.colostate.edu and provide the following information: student's name, student's CSU ID, the semester that the overload will be applied to, the maximum credit load that the student will be allowed to register for, and justification for the overload.
GS1B Graduate Application for Readmission
Form
This form is used when a student's enrollment has dropped for one or more semesters. There is a non-refundable fee of $150.00 made payable to Colorado State University. The form is filled out by the student and then forwarded to the department for approval. Once approved the form is sent to the Office of Admissions.
GS1C Change of Term Request For Applicants to Graduate Programs

This form is available for a new applicant to either update the term on his/her existing application or, if admitted, to defer to a different term. This form cannot be used with the combination of a GS7 Request for Change of Department and/or Degree Program.
Immigration Document Request Form
Form
This form is required to be submitted with all International applications to certify that the student has adequate financial support to attend Colorado State University.
GS2A Notice of Graduate Admission (Department Use Only)

A letter will be generated so that the applicant is formally notified that he/she has been admitted to Graduate School. This letter indicates the term of admission, the degree, and conditions of admission. Simply search for the student record in ARIESweb and choose "Generate Graduate Admission Letter" under the "General Student Tools" header.

GS6 Program of Study

The Program of Study is a document which lists all courses taken in pursuit of the degree as well as the graduate committee. This is the formal statement of what is done to achieve the degree, the summary of all academic planning. The Program of Study must be filed with the Graduate School before the time of the fourth regular semester registration. Students who fail to meet this requirement may be denied subsequent registration. In addition, this form must be submitted to the Graduate School prior to applying for graduation.
GS7 Request for Change of Department and/or Degree Program

Instructions
This form is used to change from one department to another or from one degree to another. A student may or may not complete the degree program from which he/she is changing. If a student has completed all coursework for the first degree, is working on research only, and wishes to gain admission to a new department or degree, it is appropriate to fill out this form.
GS9A Petition for Committee Member Changes

Instructions
This form is used to make changes to a student's committee after the student's GS6 Program of Study has been approved by the Graduate School. A student's committee must be up-to-date at the time of the preliminary examination (Ph.D. students), final examination/defense, and thesis/dissertation submission.
GS11 Study Abroad Transfer Credit Request
Form
This form is used if a student takes coursework abroad as part of his/her program and the student wants this coursework to be officially transferred to CSU.
GS14 Report of Department Examination
Form
Upon request, the Graduate School will file the results of departmental examinations in the University records. This service is provided for those departments who wish to establish a more complete and more formal indication of student completion of degree requirements.
GS16 Report of Preliminary Examination for the Ph.D. Degree

Procedures
A preliminary examination shall be administered at least two terms before the final examination/defense to determine whether the student is qualified to continue toward the doctorate. The completed and signed form must be submitted to the Graduate School Office within two working days after the results of the examination are known.
GS24 Report of Final Examination Results

Procedures
All Ph.D. students and Master's Plan A and Plan B students are required to complete and pass a final examination/defense. The examination must be held by the published deadline of the student's graduating term. The completed and signed form must be submitted to the Graduate School Office within two working days after the results of the examination are known.
GS25 Application for Graduation

Program & Specialization Codes
A student must apply for graduation by the published deadline of the student's graduating term. A student applying to graduate for the first time must submit this form.
Reapplication for Graduation
Form
An online process for a student who has applied to graduate in a previous term and who needs to update to a future term. A student must reapply for graduation by the published deadline of the student's updated graduating term.
GS25B Departmental Requirements Clearance
Form
Departmental requirements listed in Section 4 of the GS25 Application for Graduation may be cleared by completing this form. This form must be signed and submitted to the Graduate School when these requirements have been met and by the published deadline of the student's graduating term.
GS26 Request for Letter of Completion
Form
This letter can be issued to a student who has completed all of his/her degree requirements, including the posting of grades. Letters will be issued until the degree is recorded on the student's official transcripts.
GS52 Graduation Clearance Response
Form
This form can be used by a student's advisor to indicate how to resolve course discrepancies (if applicable) that were not addressed on the student's GS25 Application for Graduation. A memo or e-mail from the student's advisor can also be submitted to resolve these course discrepancies.
Thesis/Dissertation Submission
Form
This form is required of all Master's Plan A students and Ph.D. students submitting a thesis or dissertation after the final thesis/dissertation has been reviewed and approved by the student's committee. The completed and signed form must be submitted to the Graduate School Office by the published deadline date of the student's graduating term and before the electronic submission of the thesis or dissertation.

- If a student wants to drop or withdraw from a course(s), he/she will also need to submit a Late Registration Request, which is described above, along with the Registration Appeal Checklist.
- If a student wants to withdraw from all of his/her courses, the student will need to contact The Center for Advising and Student Achievement (CASA).
